Help filling out application for child support?

I have a 2 week old infant, and Im filing for CS. On the appl. it asks for monthly expenses, it asks for the amount in groceries and household supplies, would I need to enter an amount since i have wic and they give me formula? And it asks about eating out? leave it blank or what? What about laundry and cleaning and clothes? I need some help in these amounts. Whats the avergae costs? thx!
