Do I need to Include a Limited Liability Worksheet with a form 568?

This is my second year filing a LLC Return of Income CA Form 568. Last year I attempted to do it myself but wasn't sure if I was filling it out correctly, so I paid the accountant that does our personal taxes to fill it out for me. He said that the form is so easy to fill out that I should use it as a guide for this year. Well, there have been a couple small changes in the form from last year to this year, but I think I have them all figured out. The only thing I am unsure of is if I Need to Include a LLC Income Worksheet or not. My accountant included one last year with my taxes, but I can't find anyplace on the CA Franchise Tax Board's website for me to download one and fill it in. Can someone familiar with this please help me out? FYI: I am not using any tax software, I simply downloaded the forms from the CA Franchise Tax Board website.
